Bring: Proof of income, Proof of address (Driver’s License, ID, etc.), Proof of Citizenship (Driver’s License, ID, etc.)

Doctors refer clients to nearby locations for treatment

Low sliding scale; if no income, pay nothing; helps patient determine eligibility for programs: Find coverage programs available as well as sliding scale here: http://sabancommunityclinic.org/patients/coverage-programs

Patients will not be turned away due to inability to pay.
